Toulouse 2011 Application Packet Instructions: Please Retain for Reference

 

Application for NGCSU Undergraduate Admission:

  1. Print neatly using blue or black ink.

  2. Answer as follows: Status as non-degree; Term as Summer 2011; Campus as Dahlonega.

  3. Be sure to sign and date.

  4. Send application and NGCSU application fee of $30.00* to Ms. Deborah Riedmiller, NOT to NGCSU Admissions.

  • Disregard the commuter affidavit paperwork.

  • You may submit the immunization paperwork now or as late as April 1, 2011.

  • The request for your official transcript(s) and SAT/ACT scores will be made later, but here are the standards for NGCSU Joint-Enrollment admission for our program.

 

 Study Abroad Application:

  1. Complete all personal information except insurance (the program carries its own insurance).

  2. Answer as follows:  Dates overseas as June 2011; Overseas contact as Dr. Brian Mann; Sponsoring institution as NGCSU.

  3. Be sure both parent and student date and sign this form.

  4. Send application and application fees of $530.00* to Ms. Deborah Riedmiller.
     

*YOU MUST SEND TWO SEPARATE CHECKS PAYABLE TO NGCSU.  The $500.00 payment is a deposit towards the actual costs of operating the program. The $30.00 fee goes towards processing your NGCSU application. Program payments cannot, at least for the time being, be made by credit card unless you have been accepted for admission to NGCSU. Even then it involves additional cost and a 3rd party transaction. We therefore strongly recommend that you make your payment with a check.
 

Medical Self-Assessment Form:

  1. Complete this form if you wish to release information about your pre-existing health conditions and physician contact information to the directors. This information is very important should accident or illness arise during the trip.

  2. If you decline to release your medical information, you must still put your name on it and sign it in the "Decline to Complete" box. Send the form to Ms. Deborah Riedmiller.

 

Agreement and Release:

  1. Read this form carefully

  2. Be sure both parent and student date and sign this form.

  3. Send the form to Ms. Deborah Riedmiller.

 

Student Profile Form:

  1. Print neatly using blue or black ink.

  2. Answer carefully.  This information will be used to place you with a compatible host family.

  3. While a passport photo is ideal, any head shot photo that fits in the space provided will do.

  4. Send to Ms. Deborah Riedmiller.
     

Teacher/Counselor Recommendation Form:

  1. Give a form to each of two teachers or other school personnel who know you well.

  2. As a courtesy for their time, provide them with a stamped envelope addressed to Ms. Deborah Riedmiller
     

Program & Payment Policy Form:

  1. Please adhere to the schedule of payments and the policy regarding refunds.

  2. Be sure both parent and student sign and date this form.

  3. Send to Ms. Deborah Riedmiller.

  4. Retain a copy for reference.

 

Transcript or grade report

  1. Secure a photocopy of your most recent transcript or grade report.

  2. Send to Ms. Deborah Riedmiller.

  3. In February, we will need an official transcript along with your other registration materials. Graduating seniors will need to provide a final official transcript in June.
