If you believe your current language lab grade is not correct, follow these instructions:

1. Print this form.

2. Fill in the table with the following information:

   • Your name, class, and section number

   • The week number in question

   • Your current grade for that week

   • The number of minutes you believe you should have credit for

3. Go to the Lab Director (Ms. Mariana Pomphile), have her verify your attendance for that week, and then confirm it with her signature.

4. Return the form to Dr. Mann.

Remember: You are required to spend and document 50 minutes of work per week in the lab.  This means 50, not 40 or 45.
